Employee injured in fall while installing concrete forms

At approximately 10:45 a.m. on December 21, 1990, Employee #1, a carpenter of a large, single-family house under construction was installing perimeter wood forms for a concrete floor. He fell 12 ft from the second story to the first floor, sustaining head, neck, and arm injuries. Fall prevention guardrails were not in place at the edge of the floor from which Employee #1 fell.
