Investigation Summary
Employee #1 was using a very old dough cutting machine in a bakery. His left hand was resting on the flange of the chute on top of the machine. A reciprocating plate was moving up and down right next to the flange with a clearance of only 1/4 in., creating a shearing hazard. When the plate rose, it amputated Employee #1's left middle finger at the third phalanx and lacerated his left thumb.
