Investigation Summary
Employee #1 was the job superintendent on a tenant improvement job being done for an incoming tenant in a relatively new tilt-up warehouse building. Employee #1 had been supervising a job building new foundations for refrigerator units to be placed on the roof. He went by himself back up to the roof to check out the serial numbers on the existing air conditioner units. As he walked away from one of these units, he stumbled over the curb to a skylight, and fell head-first 30 feet through the skylight. The skylight was a standard Bristolite Lexan skylight, measuring 52 inches by 100 inches. Employee #1 sustained fractures to his ankles, feet, legs, and back, but survived the fall.
