Investigation Summary
At approx. 11:15 a.m. on April 25, 1993, Employee #1 of Joe Dailey and Sons was cleaning an offset printing machine with a rag without turning off the machine. He was cleaning the water roller when his left thumb got caught between the running rollers. He suffered a severe cut on his thumb. He was taken to the USC Medical Center, was hospitalized for three days, and is now being treated as an outpatient.
