Investigation Summary
At approximately 9:00 a.m. on February 25, 1991, Employee #1, the supervisor, was operating a wire wheel brush section machine, which was in automatic mode. The machine manufactured a wheel-type brush in three stages in a stamping-type motion. Mechanical arms automatically transferred the wheel brush from one stage to the next until it was finished. Employee #1 was watching over the production process when he observed some irregularly cut wires getting tangled on the steel ring of the wire brush. Instinctively, he reached in to disentangle the wires. His right index finger became caught between the two pressing parts of the machine and was amputated about 1 1/2 inches from the end.
