Investigation Summary
At approximately 7:45 a.m. on October 2, 1991, Employee #1 was assigned to cut 3/4 in. fabric with a portable circular fabric cutter. The front end of the circular blade is equipped with a guard that was adjusted to just about the height of the fabric to be cut. While cutting the fabric, Employee #1 was leaning across the table to reach the far end of the fabric. Employee #1 was pushing the portable cutter with his left hand while he pressed his right hand on the fabric. During the pushing motion, the portable cutter swirled and the blade came in contact with Employee #1's right index and middle fingers. Employee #1 sustained a flesh cut on the tip of the index finger and lacerated his middle finger.
