Investigation Summary
At about 1:20 a.m. on December 6, 1991, Employee #1, a 32-year old employee of LA Paper Box and Paper Mills, was working near a rewinder from which pairs of 23 in. diameter, 2,300 lb rolls of paper were being unloaded. One of the rolls became unbalanced, bounced back from a choke block, tipped, and fell onto Employee #1's right foot, fracturing it. Employee #1 was hospitalized in Presbyterian Intercommunity Hospital. He is now being treated as an outpatient. It was evident that Employee #1 was facing the wall and did not see the roll falling. A citation was issued and the employer was advised to enforce safety procedures to prevent future accidents.
