Investigation Summary

Investigation Nr: 170188999
Event: 01/13/1991
Employee injured when struck by falling load

At approximately 1:45 p.m. on January 13, 1991, Employee #1 and a coworker, of Dreyer's Grand Ice Cream, were using a motorized hand-rider truck to move a heating press from storage. Employee #1 was walking alongside the truck as the coworker drove. When the truck turned left down an access aisle, the load tipped and fell off, knocking Employee #1 to the ground. The press had not been secured to the truck. Employee #1 was transported to Long Beach Memorial Hospital for treatment and possible surgery. The employer was cited for a serious violation of T8CCR 3560(m), failure to secure load while moving a load of excessive length, width, or height. The employer was also advised to oversee employees' actions on the job and to enforce strict safety methods in the future.
