Employee's arm lacerated when caught in inserter machine

At approximately 12:00 noon on December 13, 1990, Employee #1, a selector for Foster-Forbes Glass Division, was filling inserts in shop inserter machine #231 when a partition jammed. She was reaching into the left side of the machine to remove it when her left arm became caught between the sliding table and the pick-up arm and was lacerated. She was hospitalized for 1 day and then treated as an outpatient. The employer was issued citations to retrain employees and to install proper guards to prevent employee exposure. The employer was also advised to practice and oversee employee operations to insure overall safety and health.
