Investigation Summary
Employee #1, a 25-year-old punch press operator for Cirillo Manufacturing Co., was punching holes using part of a Bliss 60 ton punch press. The punch press did not have any point of operation guard or double-hand controls to protect the operator from the point of operation. Employee #1's right hand was exposed to the danger zone and part of his index, middle, and ring fingers were amputated. He was taken to Community Hospital and hospitalized for three days. A citation was issued to the employer ordering him to correct the unsafe condition. The employer was also strongly advised to safeguard the machines and to train employees on accident prevention.
