Investigation Summary
Employee #1 fell about 10 feet as he was stepping off a 16-foot aluminum ladder that was set at a flatter pitch than required. He was using the ladder in the combined exhaust and emergency exit in the Wilshire/Western Station of Metro Rail Project B-231. The ladder appeared to be fairly new.
