Investigation Summary
At about 11:40 a.m. on December 30, 1993, Employee #1 was relocating a trip tank to a new location on oil Island Grism. Employee #1 works for the lease holder as a lease operator. Moving the trip tank was within the scope of his normal duties. The skid-mounted tank weighs 11,100 pounds and measures 9 feet by 9 feet by 7 feet 9 inches high. He had secured the tank to the forks of a Hyster model H250H forklift by chaining the tank to the forklift's mast. The chain that he used was a trucker's chain--not a grade 8 chain. He was installing dunnage under the trip tank when the chain securing the tank broke. The tank slipped off the forklift's forks and fell onto Employee #1's foot, causing multiple fractures and requiring surgery on the foot. Employee #1 was wearing rubber high top boots with steel toes. Forklift loads of excessive width, length, or height shall be balanced, braced, and secured so as to prevent tipping and falling.
