Investigation Summary
Contractors were remodeling a Crown Book Store in the Lakewood Center Mall. The general contractor on the job was Turner Smith Construction. Employee #1 was a subcontractor and foreman working for Exterior Wall Systems, which was installing iron furring channels from an 8 ft scaffold that was around the building. Employee #1 went to his car to get a pair of nippers. After climbing an 8 ft ladder, Employee #1 stepped from the ladder onto the planks of the scaffold. His feet became tangled in nylon drop lines. In trying to free himself, Employee #1 tripped and fell off the inside of the scaffold. Employee #1 was taken to Lakewood Regional Medical Center for treatment of a fractured hip and three fractured ribs.
