Investigation Summary
Employee #1 and two coworkers, were putting up brackets so that scaffolding could be mounted on them. When they finished installing one scaffold level, Employee #1 untied his safety lanyard to climb higher. He intended to retie his lanyard when he reached the next level, but the member he grabbed came loose and he fell 12 ft. He landed on the scaffold brackets, and suffered a compressed fracture of the spinal cord. Lack of safe access to the work area was cited as a causal factor.
