Investigation Summary
Employee #1 was atop a large portable containment tank. He was pulling hose up to the tank when he lost his balance. He fell through a 4 foot x by 3 1/2 foot opening to the bottom of the tank approximately 12 feet below. Employee #1 fractured his right hip.
