Investigation Summary
At approximately 10:10 a.m. on July 12, 1991, Employee #1 was using a farm tractor to pull a water tank trailer on a dirt road. The tractor stalled midway up an incline and started to roll back downhill. The tractor and water tank trailer jackknifed, crushing Employee #1 between them. He suffered serious injuries to his upper torso, for which he was hospitalized.
