Employee injured when struck by flying object

At approximately 9:30 a.m. on May 15, 1991, Employee #1 and three coworkers were repairing the wellhead expansion joint at well #505 in the Kern River oil field in Bakersfield, CA. When they had completed the job, they noticed that one of the Barco swivel joints had a small leak. Employee #1 picked up a small ball peen hammer and tapped the back of the ring nuts to seat the swivel joint. The leak stopped, but after a few minutes, the same joint began to leak again. Employee #1 again tapped on the Barco nuts, but this time the joint flew apart. Employee #1 was struck by the flying pieces and suffered a broken left arm and six fractured ribs. He was hospitalized for 7 days.
