Employee's hand cut in portable power saw

At approximately 11:15 a.m. on May 23, 1991, Employee #1 was using a hand-held circular power saw to cut notches in a piece of 24 in. by 15 in. by 3/4 in. plywood. He was holding the board with his left hand, with one 24 in. edge resting on the concrete floor, and making parallel cuts in the opposite 24 in. edge. As Employee #1 was making a cut, the saw bound in the wood and the blade traveled across the board, contacting Employee #1's left hand. He suffered a laceration between the thumb and forefinger, and was hospitalized. The board was a cover for a concrete form opening, and the notches were being made to allow the rebar to protrude.
