Employee's leg fractured in fall from tank

At approximately 3:25 p.m. on February 28, 1992, at the Buttonwillow Warehouse Company facility in Taft, CA, Employee #1 was attempting to remove the threaded nipple of a relief valve, which remained in the top of a 10 ft tank after the top of the valve had broken. Employee #1 inserted an easyout into the inner diameter of the nipple and turned it with a socket and ratchet assembly. To apply additional torque, Employee #1 placed a 40 in. snipe on the end of the ratchet handle. As he pulled the snipe, the socket broke, and his momentum carried him over the side of the tank. Employee #1 fell 10 ft, and fractured his leg. He was not wearing a safety belt.
