Investigation Summary
On March 28, 1991, Employee #1 was preparing to put away the bailer on rig #7. He clipped the anti-fall device onto his safety belt and climbed the rig ladder approximately 25 feet to the tubing platform. As he stepped onto the tubing platform, he disconnected his safety line from his belt and secured it to the railing. Employee #1 then went to the opposite side of the tubing platform to put away the bailer. The upper end of the bailer was meant to be grasped with both hands so as to swing the lower end over a short piece of pipe welded to the rig and used as a holster. The operator would then lower the bailer. As Employee #1 was moving along the tubing platform, he slipped and fell approximately 25 ft to the ground. He suffered broken ribs and multiple fractures of his right arm. Employee #1 did not have the safety line, which was still on the railing, attached to his belt.
