Investigation Summary
Employee #1 was adjusting a Bodine assembly machine using a hand-held jog button switch to override the presence sensor device and intermittently advance the work piece. According to Employee #1, he does this to check assembly work at various stages. When he was removing the piece from the fixture with his bare hands while still holding the single control jog switch, the switch was accidentally activated. The machine advanced, crushing and puncturing his left hand. Employee #1 was hospitalized.
