Investigation Summary
At 11:30 a.m. on June 9, 1990, Employee #1 was working for a builder/general contractor who was installing 4 by 8 plywood sheets as part of a temporary walkway designed for access to other sections of a wood joist/I-beam roof structure. He stepped on an unsecured plywood sheet and fell 20 ft off the roof to the floor. Los Angeles Fire Department paramedics responded to the scene and transported Employee #1 to Cedar Sinai Medical Center where he was hospitalized for one week with numerous fractures to his hands and wrists.
