Employee injured when struck by 60-ton jack

Employees were attempting to raise the six wheel girder of a large rail-mounted crane being constructed at a work site. The employees were trying to reposition the gudgeon pin connecting the six wheel and four wheel girders. They were using an electrically driven 60-ton Porta-Power jack. Because of the design of the girder end, the employees could not lift the girder directly, so they built a stand of 1 in. plate and 6 in. diameter pipe to mount the jack on with two loose pieces of 1 in. plate atop the pipe. The jack was placed on the stand, off center to the inboard side with the ram contacting the girder on about half of its diameter. As the lifting operation started, Employee #1 climbed onto the four wheel girder to see if the six wheel girder was moving. He heard a loud crack and the jack kicked out and struck him in the chest, breaking five ribs and knocking him from the 9 ft high girder to the ground. Employee #1 sustained a severe laceration on the top of his head. The employer was cited for improperly designing and building the jacking fixture.
