Investigation Summary

Employee #1, a supervisor, ordered an operator to raise the forks of a forklift that had been loaded with two bales of knocked-down corrugated carton boxes so that he could place a wooden pallet under them. The boxes were stacked one on top of the other and each bale weighed approximately 300 pounds. After placing the wooden pallet, Employee #1 told the operator to place the bales on the pallet. As the bales were being lowered, Employee #1 centered the pallet under the load. The operator had to tilt the mast so that he could unload the bales. The top bale fell onto Employee #1. He sustained a compressed fracture of the back.
