Employee's finger lacerated in bagel-making machine

At approximately 10:15 a.m. on June 5, 1992, Employee #1 was trying to pick up a bagel that had fallen between the chain drive and the sprocket wheel of a bagel making machine. His right index finger became caught and was deeply lacerated. The employer later stated that the guard to the chain and sprocket had been removed to facilitate cleaning and that Employee #1 had been instructed 2 weeks earlier not to reach into the unguarded area. The employer was cited for a violation of T8CCR 4075(a).
