Investigation Summary
Employee #1 had entered a 40 to 60 ft long tanker truck to use a power spray on epoxy glue that remained stuck to the sides of the tanker despite caustic sodium hydroxide and high pressure water rinses. The tank had been ventilated with air from tank wash hoses. Employee #1 was discovered unconscious in the tanker by a standby coworker who was returning from a 5 to 10 minute absence. The coworker entered the tank, but since he could not move Employee #1 up the ladder, he called the fire department. Employee #1 was found with a broken respirator, its hose wrapped around his leg. Employee #1 does not remember falling or any feeling of being overcome. He was released from the hospital in an hour. Employee #1 probably was not overcome by chemical exposure, because he showed no signs of chemical exposure or oxygen deficiency, nor did the coworker. Employee #1 most likely fell from a ladder due to a false step. No safety violations were cited.
