Employee's hand caught in pinch point

At approximately 3:30 p.m. on June 16, 1992, Employee #1 and a lead man coworker were installing a large drive spindle (approximately 30 tons) as a component of the company steel rolling mill. Employee #1 was helping align the spindle by hand as the crane operator lowered it into position. He placed his hand into a 6 in. diameter pin hole, which resulted in injuries for which he was hospitalized. Employee #1 stated in an interview that he impulsively reached his right hand in close proximity of the pinch point. The crane operator was qualified and there were no faults in the crane or rigging. Employee #1 was well qualified and he had performed the pretask safety briefing. There were no violations of T8CCR safety orders, thus no citation was issued.
