Investigation Summary
At approximately 10:00 a.m. on July 22, 1992, Employee #1 was about to pour soda ash powder into a 10 ft high, 3,000 gallon wastewater tank. Using the forks of a Toyota forklift, model #42-5FG25, serial #40496, he raised a pallet with a soda ash bag to a height of approximately 7 to 8 ft. He was attempting to climb the mast of the forklift to gain access to the pallet platform by stepping on the dashboard when the heel of his shoe hit the tilt control. The mast started tilting backward, pinning Employee #1 against the canopy frame and causing serious abdominal injuries. He was taken to Pomona Valley Medical Center for treatment. He had operated this forklift for two months and he had six years of operating experience. The employer did not have a forklift training program.
