Employee hospitalized after fall from loader bucket

At approximately 7:30 a.m. on March 5, 1992, Employee #1 was in the bucket of an 840 Allis Chalmers skip loader, model 1972. The employee was lifted 32 to 33 in. in the bucket so that she could reach the upper guide channel on the sliding door of a boxcar and open the door. There was a dent on the upper guide channel that prevented the sliding door, which was hooked to the loader bucket by a chain, from moving. This caused the loader to stop suddenly and throw Employee #1 out of the bucket and onto the asphalt floor. Employee #1 suffered serious injuries to her left wrist and thumb. She was taken to San Bernardino Community Hospital, then to Kaiser Permanente Hospital, in Fontana, where she was hospitalized for three days.
