Investigation Summary
At approximately 5:00 a.m. on August 14, 1992, Employee #1, of R.J. Anderson, Inc., was at his job station. His task was to place desktops, one at a time, onto a conveyor, which carried them into a computerized saw. Because the machine operator was not present there was nothing for him to do and Employee #1 became nervous. He reached for the microswitch near a saw blade to start the saw. When he released the switch, the saw blade activated, amputating his right thumb.
