Employee's leg injured when foot strikes box on floor

Employee #1 stepped onto a conveyor to reposition a case of product that had skewed. When she stepped off the conveyor (approximately 1 1/2 ft) her foot struck the corner of a box of product she had placed on the floor next to the conveyor before attempting to reposition the product on the skid. Her leg was fractured. There were no witnesses to the accident. No violation contributed to the accident.
