Investigation Summary

Investigation Nr: 170719181
Event: 01/20/1993
Employee's hand crushed in Van Dorn press

At approximately 4:00 p.m. on January 20, 1993, Employee #1 apparently climbed to the top of a Van Dorn press and reached into the moving press to remove a part. He did this in spite of a warning sign on the press instructing employees not to place their hands in the press. The press is equipped with electrical micro-switches and a mechanical blockout device. In addition, the ram is outfitted with a sensing device that should automatically stop the press if it meets any resistance (such as the employee's hand). Employee #1's right hand was crushed by the press. Employee #1 was taken to Los Robles Medical Center in Thousand Oaks, CA.
