Employee's hand ignited by exploding bulb

Employee #1 was flushing an automobile air conditioning system with a flammable solvent, which got on his right hand. The droplight he was using had an incandescent light bulb and was not intrinsically safe. When Employee #1 moved the light, the bulb exploded, igniting his hand. He has undergone three surgeries on his hand and spent a total of 12 days in the hospital.
