Investigation Summary
Employee #1, an experienced truck driver, was awaiting the arrival of another chop truck so that he could exchange trucks and drive to where his personal vehicle was parked at a neighboring farm and go home. Employee #1 was requested to monitor a slow moving chain and bar conveyor in the bed of a truck-mounted chop box belonging to and operated by a separate employer. After unloading the chop, the operator continued operation of the conveyor, clearing the final residue to facilitate remote closure of the doors from the cab. The operator was then to return to the harvest operation or the shop, as applicable. Employee #1, on his own initiative, and assuming without verifying it that the conveyor had stopped, attempted to inspect the slowly rotating shaft of the conveyor. An interconnecting bar on the slow moving conveyor trapped and severely traumatized Employee #1's right hand, resulting in the subsequent amputation of four fingers. Employee #1 was removed from the scene and transferred to St. John's Hospital in San Francisco, CA.
