Investigation Summary
At 11:00 a.m. on March 4, 1993, Employee #1, a welder, was welding on a 90 degree elbow section of a newly-installed 20 in. pipeline. He was lying on his back under the elbow section. As he was completing the welding, wooden cribbing, which had been erected 20 to 30 ft south of his work station and was being used to support the pipeline, slipped out of position and allowed the elbow section to fall onto Employee #1's chest and head. Employee #1 was taken by paramedics to the Long Beach Memorial Hospital, where he was pronounced dead.
