Investigation Summary
At about 6:30 p.m. on July 15, 1993, Employee #1 was cleaning a food manufacturing machine known as a divider. He and a coworker were trained in lockout procedures. They said that they turned the machine off but did not lock out the power line. When Employee #1 reached into the hopper of the divider with his index and middle fingers to remove the sticking dough, the divider blade fell and amputated the tips of his two fingers. Employee #1 was taken to an industrial medical clinic and then to the Western Hand Center, Long Beach, for treatment. A serious violation of 8CCR3314(a), a regulatory violation of 14308(a), and a serious violation of 3314(h)(3) were cited.
