Investigation Summary
At about 7:30 a.m. on July 24, 1990, Employee #1 had just lifted five aluminum logs from a laydown table with a 10,000 lb capacity forklift truck. One of the logs, which were to be stacked in the preheat log storage area, measured about 9 in. in diameter by 20 ft long and weighed 1,400 lb. As Employee #1 reached the log storage area, which was about 100 ft from the laydown table, he turned off the truck's motor, leaving the load laden forks suspended about 2 ft from the floor. Employee #1 descended from the truck to set up steel spacers on top of the existing stack of logs. While Employee #1 was setting the spacers, he failed to notice that one log had started to roll off the forklift. When the log fell, it bounced, rolled, and subsequently struck Employee #1's right heel, resulting in a serious fracture for which he was hospitalized.
