Employee's finger amputated in paint filling machine

At approximately 9:15 a.m. on October 16, 1990, Employee #1 was cleaning spilled paint on the table around the reciprocating part of a paint filling machine. The reciprocating part was very small and run by a fractional horsepower motor at low speed. Her right index finger became caught by the part and was amputated. She was treated at a local hospital and released several hours later. Employee #1 had been working at the paints and adhesives manufacturer since May 20, 1986, and was adequately trained on this and other machines. The employer stated that Employee #1 had been instructed not to clean the spills while the machine was running. The employer was to install a small plastic guard around the pinch point to prevent a recurrence.
