Employee catches fingers in running chain & sprocket drive

At about 11:30 a.m. on January 29, 1993, Employee #1 was lubricating a running chain and sprocket drive. Accidentally all the fingers on his left hand except the thumb were caught in the danger zone and the ends of the digits were amputated. He said he did not de-energize and lockout the machine, despite recent training, because he did not want to interrupt production. General violation of 8CCR 3314(a) was observed and cited. Also cited were 342(a), regulatory, and 3314(h)(3), serious.
