Investigation Summary
On November 2, 1993, at about 1:00 p.m., Employee #1, while attempting to remove two jammed pieces of wood from a woodworking machine known as an edge bender, had the tips of his left index, middle, and ring fingers amputated. Attempts to interview Employee #1 were unsuccessful. It appears that he had been instructed to seek the foreman's help rather than unjam the machine himself, and that he had followed such instruction in the past. It does not appear that he was assigned to unjam the machine or that the injury was the result of other accidental contact. No accident related violation was cited. Unrelated general, regulatory, and serious violations, including energy control procedures, were observed and cited.
