Investigation Summary
At approximately 1:00 p.m. on October 17, 1991, Employee #1 was leaning over the side of a carport nailing a slab of plank onto a beam. He slipped, lost his balance, and fell approximately 12 feet to the ground. Employee #1 landed on a stack of boards and sustained bruises and two broken ribs, for which he was hospitalized.
