Investigation Summary

Investigation Nr: 170181127
Event: 11/14/1991
Employee's leg struck by falling T-section beam

On November 14, 1991, Employee #1 was removing the bolts from a splice on two T-section beams laid about 3 1/2 inches above the floor. The beams were supported by 4 in. by 4 in. wood members and 6 in. by 8 in. metal spacers. He had finished removing 7 bolts but was having problems removing the last bolt when the 30 foot long T-section flipped over and struck him in the middle of his leg. It appears while removing the last bolt, Employee #1 jerked the end of the T-section beam, which fell on his leg. The lower end of the T-section pinched his toes, but Employee #1 wearing steel toe safety shoes and his toes were not injured. Employee #1 was transported by paramedics to Whittier Presbyterian Hospital in Whittier for emergency treatment. The employer was cited for a general violation of T8CCR 3328(e) for not adequately securing the two T-sections before working near the splice. The employer will use the crane or other suitable devices in the future before allowing employees to work near these sections.
