Investigation Summary
At approximately 10:30 a.m. on September 22, 2000, Employee #1 and another worker were using a rented boom forklift, Grandall Model Number 534B-8, Serial Number 0144752J, with an 8,000 pound capacity. They were using it to nail sheer panels, 4-ft by 8-ft by 0.75-in., on the outer wall of a house under construction. They had forgotten to bring their ladders. They were using a 4-ft by 8-ft by 2-ft high job made wooden box to raise Employee #1 up. The box weighs approximately 300 pounds and was made primarily for carrying plywood and hardware from house to house. It was not designed as a vehicle mounted work platform to carry or elevate a person. Employee #1 and the box were not secured to the forklift and the forks were not placed under the end sections of the box. Employee #1 stepped onto the unsupported section of the box and the box flipped over to the right. Employee #1 fell backwards, approximately 8 ft to the dirt ground. The box then landed on him and he suffered massive internal chest injuries and died.
