Investigation Summary
Employee #1, an air conditioner repairman, went alone to a single story building housing small businesses to make an estimate for maintaining the a/c units on the roof. He walked around to a sandwich shop and stated his head hurt and he thought he had been unconscious. His employer was called and Employee #1 was taken to the hospital. It was determined that he had a skull fracture and was hospitalized. On August 5, 1991, Employee #1 died. There was a ladder placed near the bottom of a fixed ladder at the site, but it is unknown if Employee #1 fell from the ladder, slipped on the concrete surrounding the building, or if something else injured him. There were no witnesses and Employee #1 did not remember what happened before he died.
