Employee's fingers amputated in chain drive of conveyor

At approximately 4:30 p.m. on August 20, 1990, Employee #1 shut off conveyor #105 and walked 85 ft from the operator's console to the chain drive of the conveyor. He climbed a stepladder to reach the chain drive, removed the guard, and began working on the chain. When a coworker turned on the machine, Employee #1's fingers became caught between the chain and sprockets, and three of his fingers were amputated. The employer was cited for violation of T8CCR 3314 (b).
