Investigation Summary
At about 12:00 noon on March 12, 1992, Employee #1 was operating a 22-ton press, 1-J #3, serial #1613, with a two-hand trip device with full revolution clutch. He was compressing the metal hubs on the rubber casters and was taking a formed part out when the ram suddenly came down and squeezed his hand to the part of the lower die. The press did not go down all the way; it stopped over his finger. The . Employee #1 was taken to Queen of the Valley Hospital, where he was admitted for treatment. During inspection the press was tripped several times and seemed to work satisfactorily. It is not known how the ram came down, but the employer's consultant will check it thoroughly before it is put back in service. No violations related with the accident were observed during the inspection. Employee #1 has been employed with this company since January 23, 1989, and had been trained in the safety aspects of working on punch presses.
