Investigation Summary
At 12:00 noon on February 6, 1992, Employee #1 was planing a 1/2 in. by 5 ft piece of wood stock on the Wadkin Bursgreen shaper, type 1117, serial #4. To shape a piece of stock, the employee is supposed to simply push the wood over an auger which is adjusted to cut whatever measurements are dialed in. Employee #1 placed another piece in back of the first piece to speed up the process. The first piece was cut and then kicked back toward Employee #1 due to the pressure exerted on it. Employee #1 was struck in the abdomen and hospitalized. The employee was not trained to operate the machine.
