Investigation Summary
At about 10:30 a.m. on February 25, 1992, Employee #1, of United Can Co., was walking by the pallet conveyor area when an electrician asked for his help diagnosing an electrical overload on the machine. The electrician was at the control of the machine, which was shut off. After they had finished, Employee #1 and the electrician walked away until Employee #1 went back to look at the conveyor chain one more time. He lifted the chain and at the same time the machine started running because the electric eye was interrupted. His fingertips were caught in the sprocket and chain. Employee #1 lost two fingertips of his right hand. He was taken to Martin Luther King Hospital for treatment. Employee #1 said that he had been trained in lockout/tagout procedures and had attended many safety meetings on the procedures. In fact, he is a member of the safety committee. He said that the accident was his fault and he knew better than to do what he did. There was no accident-related violation.
