Investigation Summary

According to a contractor, at about 9:30 a.m. on December 6, 1991, Employee #1, of R-Jay Custom Electric, Inc., was descending a 10 ft wooden ladder at a single family home in Sierra Madre when he fell 8 ft and struck his head against the cement floor. Employee #1 was taken to Arcadia Methodist Hospital by paramedics and hospitalized for three days. He was released to recuperate at home. An accident inspection was conducted the day of the accident. On December 20, 1991, a meeting was held with the contractor. The ladder was inspected and no defects in the A frame were observed. When interviewed over the phone about the accident, Employee #1 stated that he fell off the ladder while descending, but did not remember exactly what happened. No safety violations were observed.
