Employee run over and killed by waterpull

At approximately 2:10 p.m. on Sunday, June 21, 1992, Employee #1 came to a job site where a heavy equipment operator had been assigned to operate a waterpull, which controls dust. Employee #1 was eager to learn how to operate the equipment. He had observed the operator on Friday by following the heavy equipment in his pickup truck. Employee #1 was standing on the waterpull's step and holding on when the waterpull hit a bump. Employee #1 fell to the ground and was run over and killed. The employer had no way of knowing that Employee #1 would be at the job site that day.
